HMOs and Rates by County

The following table shows the 62 counties in New York State. By selecting the appropriate county link, you will be able to view the premium rates, HMO contact numbers, and participating HMOs in that county.

Each HMO contracts with participating providers in your area. To find out if your doctor participates in an HMO’s network, please see our list of HMO Provider Directories, or contact the HMO at the phone number listed for your county.

• The rates presented are the monthly rates for the Healthy NY product as of January 1, 2010.
• Rates may vary depending upon the month in which you enroll.
• To verify the rates listed, please contact the applicable HMO or your insurance broker for more information.

Note:  The "HDHP" column header abbreviation within the rate tables of each county page stands for "High Deductible Health Plan."
