What is a PDF and who should use PDF forms?
The Portable Document Format is a file format that is compatible with various types of computers and operating systems (i.e. DOS, Windows, Mac, UNIX, etc.). Forms created in this format (usually with a PDF extension) can be viewed and printed from a workstation using Adobe Acrobat software from Adobe Systems. Versions of Acrobat run on DOS, Windows, Mac and UNIX. If you have Acrobat 5.0 or a more advanced version already installed on your personal computer, you will be able to download our documents in PDF format. This will allow you to view the form and print it on any printer that is configured to work with your installed operating system.
Updated! Note: Our PDF files are best viewed and printed using Adobe Acrobat Reader 7.x or above.
If you already have the Adobe Acrobat Reader, see our selection of available downloads.
If you do not have the
Adobe Acrobat Reader, you will need to download and install this free
Reader in order to view and print PDF files. Click on the icon to begin
this process. Please read the software license agreement before downloading
the program.
